Our CalFresh Outreach Team at Center for Healthy Communities is made up of 10 Outreach Specialists. cWe are passionate about working with our local communities to meet their basic needs including food and utility access. Our team can assist with application submission for two programs; CalFresh, a food assistance program that if eligible, one could receive a monthly allotment towards purchasing food and LIHEAP, a utility assistance program that allows households to receive a benefit that goes towards paying their utility bill once a year.
We are able to assist our clients over the phone or in-person at local outreach events including Farmers’ Markets. Our team is also present at local Farmers' Markets offering support to clients using their CalFresh benefits and providing education on utilizing Market Match. Our Market Match program allows CalFresh recipients to be matched $1 for $1 up to a daily maximum of $10 to $15.

Disaster CalFresh (D-CalFresh), federally known as the Disaster Supplemental Nutrition Assistance Program (D-SNAP), is a way to meet the temporary nutritional needs of disaster victims within a 30-day period, following a natural disaster such as a flood, fire, earthquake or any other natural disaster.
